02 Program Licensing +
Organizations can license select standardized Resolute programs that may include workbooks, implementation guides, staff materials, workflows, training resources, reporting templates, and ongoing updates. Organizations needing a custom program engage Program Design and Implementation instead.

03 Care Management Services +
Care management programs can be structured in two ways depending on the organization, population, staffing model, and billing arrangement.
Direct Care Management

Resolute provides structured services for eligible Medicare beneficiaries, including senior living and assisted living partnerships.

Contracted Care Management Support

Resolute may provide appropriate staffing, workflows, documentation infrastructure, training, reporting, implementation, or program management support to practices that maintain their own clinical and billing structure.

Medicare eligibility, consent, coverage, documentation, supervision, billing requirements, and applicable beneficiary cost sharing still apply.

For Individuals and Referral Partners

Focused Clinical Evaluations

Resolute provides defined, one time clinical evaluations for individuals and for providers who need a reliable evaluator. Each service ends with a written report you can use. These services are separate from ongoing medication management.

  1. Reach out or send a referral. Individuals can use the intake form. Providers can submit online, fax, or email.
  2. Send any required paperwork. If an organization has given you forms, send them first so we can confirm we meet their standard.
  3. Schedule. Most visits are virtual. You will need to be located in Louisiana or Texas at the time of your appointment.
  4. Receive your written report. Referring providers receive a signed report within 3 business days.
