Services may include transitional care management, acute recovery support, chronic care support, care advocacy, family navigation consults, and medication support and guidance.
Who do we work with?
We work with individuals, families, caregivers, and referral partners who need support navigating behavioral health needs, treatment changes, care coordination challenges, or transitions after hospitalization or crisis.
How do I get started?
Getting started is simple. Complete our referral or contact form, and someone from our team will reach out to discuss your needs, answer questions, and help determine the best next step.
Who can submit a referral?
Yes. We welcome self-referrals, family referrals, and referrals from healthcare professionals, therapists, and community partners.
Do you accept insurance?
Resolute Healthcare does not currently accept insurance. Services are offered through private pay and select referral or partner arrangements. If you are unsure which option may be the best fit, contact us and we can help guide you through the next steps
What is a family navigation consult?
A family navigation consult is focused support for families who need help understanding care options, identifying next steps, and creating a practical plan for ongoing support.
What is transitional care management?
Transitional care management is short-term support designed to help individuals stay connected to care after hospitalization, crisis, or major treatment changes. This may include guidance, coordination, follow-through support, and help navigating next steps
Do you provide medication management?
Resolute provides medication support and guidance, including education and care coordination related to medication routines, barriers, and continuity of care.
How quickly will someone respond?
We aim to respond as quickly as possible, typically within 1 to 2 business days.
Is Resolute an emergency service?
No. Resolute Healthcare is not an emergency service. If there is immediate risk or a mental health emergency, call 911 or 988.
