You should not have to guess what help costs while you are trying to get well. Every plan below is led by a licensed clinician and can begin within days, not weeks. Whether you are managing your own care or someone is helping you manage it, you are in the right place.
Navigation Initial Assessment
The right first step for most people. A 90 minute session, virtual or in person, where we review your medications, appointments, providers, and everything you are carrying. Within 3 business days you receive a written Care Action Plan: what to do, in what order, and what we can take off your plate. You are welcome to bring a family member or caregiver with you.
Tier 1
Steady Support
Ongoing navigation for anyone who wants a clinical partner in their corner. Includes two scheduled check ins each month, appointment and medication tracking, communication with your providers, and secure messaging in between.
Tier 2
Coordinated Support
A step up when you are in transition, especially after a hospital or facility discharge, when things most often fall apart. We reconcile your medications, schedule and confirm follow ups, coordinate across your providers, and stay in close contact with you and your care team.
Tier 3
Full Navigation
Our most complete level of support when medical and psychiatric needs are layered and hands on. Includes weekly contact, coordination across every provider, virtual attendance at key appointments, updates to anyone you choose to involve, and priority response when something changes.
