Nurse-led support coordination for families of children and youth on Medicaid. We connect your child to the care they need — therapies, behavioral health, ABA, equipment, school supports, and community resources — and handle the paperwork, prior authorizations, and follow-up, so you don’t have to.
Who it’s for
Children and youth from birth to 21 who have Louisiana Medicaid and a developmental, behavioral, or chronic health need — including those on the Developmental Disabilities Request for Services Registry. Support coordination is completely optional and never affects your child’s Medicaid benefits.
